While investigating the lair of the Grongi to take out the resting N-Gamio-Zeda, she and her group are overwhelmed by the miasma caused by the Grongi leader's revival. However, Yusuke is not able to save her in time, as the poison she breathed in later killed her. Prior to her death, she makes Yusuke promise that he will fight in order to protect everyone's smiles. Later in episode 12, a scientist and the creator of the G-Series of Kamen Riders from World of Agito named Toko Yashiro appears and have the similarities to Ai. Compared to her, Toko is more high-strung and sees the G3-X Suit as humanity's only hope.

Toko also has some feelings towards Shouichi as Ai had for Yuusuke, which was her motivation for building the G3-X suit. She monitors the condition of the operator and the G3 suit during combat from the G-Trailer mobile command base. Though was dead, but her grave makes a cameo appearance where Yusuke revives along with his destroyed world. Yusuke somehow find himself beside Ai's grave and departs to Tsukasa's world in a quest to revive him.
